FEBRUARY 1989 The unit entered the reporting period in Mode 5 with the RCS level at half loop.

The unit ended the reporting period in Mode 4 with the RCS temperature at 339'.

Filling of RCS was stopped with Pressurizer level at 39K.
Began Integrated Leakage Rate Test (ILRT) of the containment.
Successfully completed the ILRT.
A RHR System safety valve lifted prematurally resulting in the Pressurizer Relief Tank rupture disc failing discharging approximately 355 gallons of reactor coolant to the containment floor.
RCS fill and vent procedure completed.
Started RCS cooldown in preparation to lower the RCS level to half loop for repairs to a safety injection system check valve due to leakage in excess of tech spec limits.
RCS level at half loop.
Began RCS fill and vent.
RCS fill and vent procedure completed.
RCS entered Mode 4.
