MONTHLY OPERATING ACTIVITIES -

NOVEMBER 1988 For the period of Nov. 01 through Nov. 22, the Unit remained at the 90%

admini strati ve power 1 imit.

At 2303 hours0.0267 days <br />0.64 hours <br />0.00381 weeks <br />8.762915e-4 months <br /> on November 23, the Unit tripped.

The trip is thought to have been caused by a spurious underfrequency signal generated in the Solid State Protection System (SSPS).

An "Unusual Event" was declared because all four reactor coolant pumps tripped with the Unit (expected with an underfrequency trip signai).

The "Unusual Event" was exited 2-1/2 hours later following the successful restart of two reactor coolant pumps.

The generato~

was paralleled at 1931 hours0.0223 days <br />0.536 hours <br />0.00319 weeks <br />7.347455e-4 months <br />.

The administrative 9OX power limit was attained at 1242 hours0.0144 days <br />0.345 hours <br />0.00205 weeks <br />4.72581e-4 months <br /> on November 27th.

The reporting period ended with the unit operating at the 90$ administrative power limit.

Gross electrical generation for the month of November, 1988 was 610,880 WH.

DETAILS 11-05-88 0100 Steam was removed from the Moisture Separator Reheaters (MSRs) due to excessive leakage on the right MSR.

The MSR will ~emain out of service until the refueling outage.

11-23-88 2303 Reactor tripped due to spurious underfrequency-RCP-Buses signal

2320 An "Unusual Event" was declared due to the tripping of all four reactor coolant

pumps,

.11-24-88 0154 The "Unusual Event" was exited folIowing the restart of two reactor coolant pumps.

11-25-88 0222 The reactor was taken critical.

1230 Reactor entered Mode 1; 1931 Generator paralleled to the system.

11-26-88 1345 Stopped power ascension at 48% for chemistry cleanup.

11-27-88 0010 Commenced power ascension to 905.

1242 Reactor power reached 90%,

IA XXXXXX A reactor trip occurred due to a spurious underfrequency-RCP-Buses signal from the solid state protec-tion system (SSPS).

All four reactor coolant. pumps tripped from this spur ious underfrequency signai, The exact cause of the spurious signal could not be determined.

The suspected cause was an intermittent failure of a card in the SSPS.

The cards that could have caused the spurious signal were replaced.

The unit vlas returned to service on 11-25-88 and reached 90K power (administrative power limit) on 11-27-88.
