A. Hopkins of this office on September
8 through October 23,
1987, of
activities at
D.
C.
Cook Nuclear Plant, Units
1 and
2 authorized
by
NRC
Operating
Licenses
No.
DPR-58 and
No.
DPR-74 and to the discussion
of our
findings with Mr. B. Smith, Jr.
and others of your staff at the conclusion of
the inspection.
The enclosed
copy of our inspection report identifies areas
examined during
the inspection.
Within these
areas,
the inspection consisted of a selective
examination of procedures
and representative
records,
observations,
and
interviews with personnel.
No violations of NRC requirements
wer e identified during the course of this
inspection.
However, significant deficiencies
were identified in your
Licensed Operator Requalification Training Program.
These deficiencies
which
were initially identified through
NRC requalification
exams resulted in the
issuances
of two Confirmatory Action Letters,
CAL-RIII-87-012 and
CAL-RIII-87-012, Amendment
1, dated July 15 and September
4, 1987, respectfully
(these
CALs are included at Attachment
1 in this report).
While the specific
short-term
items of the
CALs were addressed
and closed in NRC Inspection
Reports
315/87017;
316/87017,
and 315/87024;
316/87024,
additional related
deficiencies
have
been identified during this inspection.
The deficiencies,
as
documented
in this report require that improvements
be made to your requali-
fication program.
The majority of the identified deficiencies
were addressed
in an
IMP letter,
dated
November 5,
1987, from Mr.
W. A. Nichols to Mr. T. Burdick (see
Attachment 3).
However, several
deficiencies
were not addressed;
these
include
the level of knowledge tested in the written examination process,
the level of
SRO training and the potential for exam compromise.
To enable
us to adequately
review and evaluate
the upgrades
to your training program,
we request that you
submit within 30 days, your planned corrective action addressing
these
additional areas.
In accordance
with 10 CFR 2.790 of the Commission's
regulations,
a copy of
this letter and the enclosed
inspection report will be placed in the
NRC
Public Document
Room.
