conducted
by Messrs.

F. Gill and
W.
W.
Ogg of this office during the period July 16 through August 31,
1987,
of activities at D.'C.
Cook Nuclear Plant, Units 1 and 2, authorized
by
NRC
Operating
Licenses

DPR-58 and

DPR-74 and to the discussion
of our
findings with Mr.
W.
G. Smith, Jr.,
and others following the inspection.
The enclosed
copy of our inspection report identifies areas
examined during
the inspection.
Within these
areas,
the inspection consisted
of a selective
examination of procedures
and representative
records,
observations,
independent
measurements,
and interviews with personnel.
The inspection
included
a review
of findings from an investigation
conducted
on October 22,
1986,
by
representatives
of the Department of Health and Environmental Control,
State of South Carolina, of a waste
shipment from the D.C.
Cook Nuclear Plant,
upon arrival of the shipment at the Barnwell, South Carolina low-level waste
burial site.
The results of the State's
investigations
were conveyed to your
company
by a letter dated October 27,
1986,
from the State of South Carolina.
During this inspection,
certain of your activities appeared
to be in violation
of NRC requirements,
as specified in the enclosed
Item
No.
1 has already
been the subject of enforcement action by the State of South
Carolina.
The review of your response
to the State of South Carolina regarding
Item No.
1 showed that the actions described
in Section
14 of the attached
report
had been
taken to correct the identified violation and to prevent
recurrence.
Consequently,
no reply to this violation is required
and
we have
no further questions
regarding this matter at this time.
If our description
of your corrective actions is incorrect,
please notify us in writing within
30 days of the date of this letter.
A written response
is required for Items
No.
2 through
No.
5.
We are concerned that the Radiation Protection
Improvement
Plan which you
implemented over the last two years
has not adequately
corrected
weaknesses
in your radiation protection program.
We base this concern
on findings of
recent
NRC inspections
as well as recent special
studies
conducted
by you and
your consultants.
These findings appear indicative of management
control
weaknesses
affecting your radiation protection program, particularly staffing
and organizational
weaknesses
and
a pervasive failure to implement effective

corrective actions for identified problems.
In order to address
our concern,
please
plan to meet with Region III management
representatives
within 45 days
of the date of this letter.
At that meeting,
you are requested
to present
your
program, with goals
and scheduled
milestones,
to resolve
the aforementioned
weaknesses.
In accordance
with 10
CFR 2.790 of the Corrmission's regulations,
a copy of
this letter, the enclosures,
and your response
to this letter will be placed
in the
NRC Public Document
Room.
The responses
directed
by this letter and the accompanying
Notice are not
subject to the clearance
procedures
of the Office of Management
and Budget
as required
by the Paperwork
Reduction Act of 1980,
PL 96-511.
We will gladly discuss
any questions
you have concerning this inspection.
