COOK BREAKER FAILURE On October 29, 1985, a Westinghouse DB50 breaker failed to trip at the D.
C.
Cook Unit 2.
An AIT was dispatched from Region III and this office.
Based on the AIT findings, the faulted breaker undervoltage trip attachment was returned to Westinghouse for tests under the control of the Cook licensee.
As a result of these tests, Westinghouse has produced the report WCAP 11092 "Investigations on Undervoltage Trip Attachments for Cause of Nisoperation at D. C.
Cook 2 on 10/29/85".
By enclosed letter dated September 23,
- 1987, we have notified the licensee for D. C.
Cook that the NRC will pursue the breaker failure directly with Westinghouse.
Our contractor, Franklin Research
- Center, has evaluated the WCAP 11092 and copies of WCAP 11092 and the FRC technical evaluation are also enclosed.
As we had discussed, the Vendor Inspection Branch will assume responsibilityfor any further review and direct contacts with Westinghouse.
