During the'eek of June 22,
- 1987, NRC requalification exams were administered to licensed operators at the D. C.
Cook plant.
Fifty three percent of the examinees failed.
In accordance with NUREG 1021 regarding requalification program evaluations, it is our determination that the D. C.
Cook requalifica-tion program is unsatisfactory.
You were informed of this by telephone on July 15, 1987.
A Confirmatory Action Letter to you was written to delineate commitments made in this matter on July 15, 1987.
On July 24,
- 1987, a meeting was held between our respective staff members to discuss the examination results and review your plans to implement short and long term corrective actions to the D.
C.
Cook requalification program.
Based on this meeting it is our understanding that you will:
1.
Remove from licensed duties those persons who failed the NRC requalification exams until they successfully complete a
NRC approved accelerated upgrade training program or an NRC examination.
2.
During the evaluation of the annual requalification examination to be administered in August 1987 make an assessment to determine the need for remedial training for all remaining licensed staff personnel in the area of instrumentation and controls.
3.
Provide NRC Region III with the facility exams to be administered in conjunction with Items 1 and 2 in advance for review.
