NOTICE OF VIOLATION Docket No.
50-355'ocket No. 50-316 American Electric Power Service Corporation I
As a result of the inspection conducted on October 1-31,
- 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October.7, 1980),
the following violations were identified:
l.
Unit 1 Technical Specification 3.3.1.1 and Table 3.3-1 requires the
'perability of the Overtemperature Delta-T and Overpower Delta-T Reactor Trip Channels and further specifies actions to be taken when one channel is inoperable to allow further power operation including, the inoperable channel is placed in the tripped condition within one hour.
Contrary to the above, on July 23, 1981, at about 1316 the licensed control room operator in Unit 1 noted that the loop 4 T-Averaqe, Delta-T and T-Cold narrow range had failed, and power operation con-tinued without compliance with the action requirements until about 1455.
This is a Severity I,evel IV violation (Supplement 1.D).

Unit 2 Technical Specification 6.9.1.9 states in part:
"The types of events listed below shall be the subject of written reports to the Director. of the Regional Office within thirty days of the occurrence of the event.
...b.
Conditions leading to operation in a degraded mode permitted by a limiting condition for operation or plant shutdown required by a limiting condition for operation."
Contrary to the above, a degraded bus voltage relay (62-1"T-llA) was found out of tolerance on June 12, 1981 and was not reported until August 18, 1981.
This is a Severity I,evel VI violation (Supplement 1.F).
