NRC INSPECTION REPORT 50-528/98-15; 50-529/98-15; 50-530/98-15
Dear Mr. Levine:
From May 21 through June 30, 1998, a reactive inspection was performed at your Palo Verde
Nuclear Generating Station, Units 1, 2, and 3, reactor. facilities following a Unit 1 letdown line
crack and leak of reactor coolant. Additional in office inspection was performed, at varying
periods, from July 1 through August 6, 1998, to r'eview additional information provided by your
staff regarding the ongoing equipment root cause of failure evaluations.
The results of this
inspection were discussed with you, and other members of your staff, during a telephonic exit
meeting on August 6, 1998. The enclosed report presents the scope and results of that
inspection.
The inspectors determined that appropriate prompt corrective actions were taken by licensee
personnel for the event.
In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of this letter and its
enclosure willbe placed in the NRC Public Document Room (PDR).

with you.
