SUBJECT: NRC INSPECTION REPORT 50-528/98-03; 50-529/98-03; 50-530/98-03 AND
Dear Mr. Levine:
From March 8 through April 18, 1998, a routine inspection was conducted at your Palo Verde
Nuclear Generating Station, Units 1, 2, and 3 reactor facilities. The enclosed report presents the
scope and results of that inspection.
During the 6-week period covered by this inspection, your conduct of activities at the Palo Verde
facilitywas generally characterized by safety-conscious
operations, sound engineering and
maintenance practices, and careful radiological controls; however, violations of NRC
requirements were Identified.
The first violation involves the failure to obtain appropriate authorization prior to initiation of work
on the emergency diesel generator.
The second violation represents
a failure to provide
appropriate procedural criteria for determining the minimum level in the transfer canal for
movement of spent fuel.
These violations are cited in the enclosed Notice of Violation (Notice) and the circumstances
surrounding the violations are described in detail in the enclosed report. Please note that you
are required to respond to this letter and should followthe instructions specified in the enclosed
Notice when preparing your response.
The NRC'will use your response,
in part, to determine
whether further enforcement action is necessary to ensure compliance with regulatory
requirements.
In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of this letter, its
enclosure(s),
and your response will be placed in the NRC Public Document Room (PDR).
Should you have any questions concerning this inspection, we will be pleased to discuss them
with you.
