Garrett (Docket Number 55-50345; License Number SOP-50212-2) no longer needs to maintain a license. In accordance with 10 CFR 50.74, APS is providing notification that Mr. Garrett has terminated his employment with APS, and requests that this individual's license be expired per 10 CFR 55.55.

10 CFR 50.74: Notify NRC within 30 days of the following in regard to S/RO:

(a):

permanent reassignment from the position for which the licensee has certified the need for a licensed S/RO; (b): termination of any S/RO; or (c): permanent disability or illness 2.

10 CFR 55.55(a): Each S/RO license expires 6 years after the date of issuance, upon termination of employment with the licensee, or upon determination by the licensee that the licensed individual no longer needs to maintain a license.
