AND
This refers to the inspection
conducted
by Hr. K. E. Johnston
and others of
this office on August
13 through September
23,
1995,
at the Palo Verde Nuclear
Generating Station,
Units 1,
2,
and 3, facility.
The purpose of the
inspection
was to determine
whether activities authorized
by the license
were
conducted safely
and in accordance
with NRC requirements.
At the conclusion
of the inspection,
the findings were discussed
with those
members of your
staff identified in the enclosed
report.
Areas
examined during the inspection
are identified in the report.
Within
these
areas,
the inspection consisted of selective
examinations of procedures
and representative
records,
interviews with personnel,
and observation of
activities in progress.
The enclosed report discusses
two issues
that highlight NRC concerns
with the
effectiveness
of the Palo Verde engineering organization
in resolving
technical
problems.
Both of these
issues
appear to indicate
a problem with
proper coordination of maintenance
engineers
with their system
and design
engineering
counterparts,.
In one instance,
clogging of drain lin'es caused
water to back into charging
pump lubricating oil on two occasions,
and
engineering failed to initiate
a timely or thorough evaluation of the cause of
the drain clogging problem.
Furthermore,
a condition report documenting
the
problem was not appropriately prioritized in accordance
with Palo Verde
procedures.
This appears
to be in violation of NRC requirements,
as specified
in the enclosed
Notice of Violation (Notice),
and is of concern since it
appeared
to contribute to the observed
engineering
evaluation
problems.
In another
instance,
engineering
personnel
did not perform
a thorough
evaluation of ground fault relay failures in 480 volt motor control center
breakers.
Although
a schedule for relay replacement
had
been established,
the
evaluation for replacement
did not consider risk significance or balance of
plant applications,
and the replacement
schedule
was allowed to slip without
additional
assessment.
You are required to respond to this letter
and should follow the instructions
specified in the enclosed
Notice when preparing your response.
In your
response,
you should document
the specific actions
taken
and
any additional
actions
you plan to prevent recurrence.
Your response
may reference
or
include previous docketed
correspondence, if the correspondence
adequately
addresses
the required
response.
