REPORT 50-528/95-12;

50-529/95-12;

50-530/95-12

AND

NOTICE Of VIOLATION

This refers to the inspection

conducted

by Hr. K.

E. Johnston

and others of

this office on May 21 through July 1,

1995.

The inspection

included

a review

of activities authorized for your Palo Verde Nuclear Generating Station,

Units

1,

2,

and 3, facility.

At the conclusion of the inspection,

the

findings were discussed

with you and

members of your staff,

as identified in

the enclosed

report.

Areas

examined during the inspection

are identified in the report.

Within

these

areas,

the inspection consisted of selective

examinations of procedures

and representative

records,

interviews with personnel,

and observation

of

activities in progress.

The purpose of the inspection

was to determine

whether activities authorized

by the license

were conducted

safely

and in

accordance

with NRC requirements.

Based

on the results of this inspection,

certain licensed activities appeared

to be in violation of NRC requirements,

as specified

in the enclosed

Notice of

Violation (Notice).

The violation involved the failure of individuals to

comply with surveillance testing procedure

requirements

for testing reactor

coolant

system

boundary

check valves.

This violation is of concern

because

licensee

personnel

failed to refer to the procedure prior to performing the

test.

As

a result,

approved test requirements

were not implemented

which

could have invalidated the test results.

The

NRC is also concerned

that licensee

management

failed to ensure

appropriate

root cause

evaluation

and resolution of a Unit

1 auxiliary

feedwater turbine governor problem.

In particular,

although

Palo Verde

has

experienced

previous

problems with the turbine governor valve

on this

important safety component,

and recent industry experience

has

emphasized

the

need for careful attention to these

components,

licensee

personnel

did not

pursue

a thorough evaluation of the problem until prompted

by

NRC inspectors.

We note that the revised

Emergency Operating

Procedures

were

implemented

near

the

end of the inspection

period

and are

encouraged

with increased flexibility

the procedures

appear

to provide.

We will assess

the effectiveness

of the

revised

procedures

during future licensed operator examinations.

You are required to respond

to this letter

and should follow the instructions

Notice when preparing your response.

In your

response,

you should

document

the specific actions

taken

and

any additional

such information, you should clearly indicate the

specific information that you desire

not to be placed

in the

PDR,

and provide

the legal basis to support your request for withholding the information from

the public.
