This refers to the integrated
assessment
team inspection conducted
by
Hr.
Elmo E. Collins of this office on September
27 through October
7,
1994.
The inspection
included
a review of activities authorized for your Palo Verde
Nuclear Generating Station,
Units l. 2,
and 3, facility.
At the conclusion of
the inspections
the findings were discussed
with you and those
members of your
staff identified in the enclosed
report.
Areas
examined during the inspection
are identified in the report.
This
inspection
focused
on engineering'aintenance.
and radiological controls.
Within these
areas.
the inspection consisted of selective examinations of
procedures
and representative
records,
interviews with personnel.
and
observation of activities in progress.
The purpose of the inspection
was to
determine whether activities authorized
by the license were conducted safely
and in accordance
with NRC requirements.
The results of the inspection are documented
in the executive
summary
beginning
on page iv in the enclosed
report.
Overall, the team found that
Palo Verde had
a good self-assessment
capability.
Performance
issues
were
known and were usually effectively addressed.
The team considered it
noteworthy that the Nuclear Assurance
Department
had identified
a programmatic
weakness
in the effectiveness
of site corrective actions.
Additional steps
were being taken by you to address this weakness.
The team's
independent
review also found examples of this weakness.
The team was not able to verify
the effectiveness
of these additional
steps
since they were early in the
process of implementation.
The team also considered it noteworthy that
historic problems
such
as control
room deficiencies.
operator work arounds.
and the number of drip catches
were recently given priority for reduction.
During this inspection.
the team identified that not all required information
was included
on the radiological controls area vehicle.
equipment,
and
material
release
log.
This was
a violation of procedures.
This violation is
not being cited because
the criteria in paragraph
VII.B.1 of Appendix
C to
10 CFR Part 2 of the
NRC's "Rules of Practice."
were satisfied.
In accordance
with 10 CFR 2.790 of the NRC's "Rules of Practice,"
a copy of
this letter and its enclosures will be placed in the
NRC Public Document
Room.
