NOTICE OF DEVIATION Arizona Public Service Company Palo Verde Nuclear Generating Station Dockets:

50-528 50-529 50-530 Licenses:

NPF-41 NPF-51 NPF-74 During an NRC inspection conducted on September 4 through October 15,

1994, one deviation of your Generic Letter (GL) 88-05 commitment was identified.

In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Action," 10 CFR Part 2, Appendix C, the deviation is listed below:

GL 88-05 specified that the licensee implement a program consisting of systematic measures to ensure that boric acid corrosion does not lead to degradation of the assurance that the reactor coolant pressure boundary will have an extremely low probability of abnormal

leakage, rapidly propagating failure, or gross rupture..

The program should include a determination of the principal locations where leaks that are smaller than the allowable technical specification limit could cause degradation of the primary pressure boundary by boric acid corrosion and a method for locating small coolant leaks.

In a response letter to the U.S. Nuclear Regulatory Commission dated Nay 27,

1988, the licensee committed to determine principal locations where a

small leak could cause degradation of primary pressure boundaries and to incorporate this list into a procedure for visual inspections.

Contrary to the above, as of October 15, 1994, the licensee's procedure for implementing the GL 88-05 program, "Visual Examination for Leakage,"

(Procedure 73TI-9ZZ13, Revision

7) did not incorporate the list of principal locations where a small leak could cause degradation of primary pressure boundaries.

76011, and a copy to the NRC Resident Inspector at the facility that is the subject of this Notice, in writing within 30 days of the date of this Notice, the reason(s) for the deviation, the corrective steps which have been taken and the results
achieved, the corrective steps which will be taken to avoid further deviations, and the date when your corrective action will be completed.

Where good cause is shown, consideration will be given to extending the response time.
