This refers to the inspection

conducted

by Mr. K. Johnston

and others of this

office on July 24 through September

3,

1994.

The inspection

included

a review

of activities authorized for your Palo Verde Nuclear Generating Station,

Units I, 2,

and 3, facility.

At the conclusion of the inspection,

the

findings were discussed

with those

members of your staff identified in the

enclosed

report.

Areas

examined during the inspection

are identified in the report.

Within

these

areas,

the inspection consisted of selective

examinations of procedures

and representative

records,

interviews with personnel,

and observation

of

activities in progress.

The purpose of the inspection

was to determine

whether activities authorized

by the license

were conducted

safely

and in

accordance

with NRC requirements.

The results of this inspection

are

documented

in the enclosed

report.

Early in the inspection period,

the "reengineered"

organization

was

implemented for the maintenance,

engineering,

nuclear assurance,

and licensing

organizations.

While we noted that the transition

had occurred

smoothly

and

that

some benefits of the

new organization

were realized,

we also noted that

there

were lingering concerns

at the maintenance

worker level with the

effectiveness

of the

new organization

and the work processes.

While we

recognize that

such

broad

changes will result in some worker anxiety,

we

encourage

you to continue to thoroughly assess

all issues

as they

may be early

indications of weaknesses

in the

new work processes

and organizations.

You identified that shift technical

advisor

(STA) manning requirements

were

not met for a period greater

than two hours

as

a result of the shift technical

advisor's unfamiliarity with Technical Specification

Requirements.

This

violation is not being cited because

the criteria in paragraph VII.B.2 of

Appendix

C to

10 CFR Part 2 of the

NRC's "Rules of Practice,"

were satisfied.

In addition, it was identified that required presentations

to the Plant

Board were not made in accordance

with procedures.

This violation is also not

being cited because

the criteria in paragraph

VII.B.I of Appendix

C were

satisfied.

In both instances,

your staff took prompt

and effective action to

correct the problem.

We were also encouraged

by the prompt actions

and

responses

to other inspector observations

and findings.

Three unresolved

items were identified concerning the:

failure of 'the Unit 3

auxiliary feedwater

pump to start

(Section 2.4); failure of a containment

spray valve to operate

(Section 3.2);

and environmental qualification of

auxiliary feedwater

steam

supply valves

(Section 6.2).

In accordance

with 10 CFR 2.790 of the NRC's "Rules of Practice,"

a copy of

this letter

and its enclosures will be placed in the

NRC Public Document

Room.
