Palo Verde Nuclear Generating Station (PVNGS)
Unit 3 Docket No. STN 50-530 Steam Generator Inspection Report File: 94-056-026 This letter is being provided to transmit the PVNGS Unit 3 Steam Generator Report (enclosed).
Unit 3 was shut down on March 19, 1994 to begin its fourth refueling outage (U3R4). The Unit 3 steam generators were inspected during the refueling outage using Eddy Current Testing (ECT) technology comprised of both bobbin coil probe and motorized rotating pancake coil (MRPC) probe.
The inspection identiTied a total of 17 tubes with single axial indications (SAls) in the upper bundle area, one tube in steam generator 31 and 16 tubes in steam generator 32.
In addition, six tubes in steam generator 31 and eight tubes in steam generator 32 were also plugged for a total of 31 tubes being plugged during this outage.
There were no circumferential indications found by ECT inspection during this period.
Each steam generator was also full bundle chemically cleaned and sludge lanced.
During the startup process the steam generators were boric acid crevice flushed and at low power were boric acid soaked.
Ethanolamine treatment will be implemented to reduce iron transport to the steam generators.
PVNGS has implemented a molar ratio control program which includes reduction of contaminants (sodium and sulfates), while using ammonium chloride to maintain proper Na/Cl ratios.
These extensive and aggressive corrective actions have been taken to control the crevice environment in the steam generators.
Based on the corrective actions and inspection
- results, it can be concluded that operation of Unit 3, until the next scheduled mid-cycle outage, will not constitute a safety concern.
