This refers to the inspection 'conducted

by Messrs. I. Barnes,

L. Ellershaw,

and

W. Walker of this office on April 19-29

and

May 9-13,

1994.

The

inspection

included

a review of activities authorized for your Palo Verde

Nuclear Generating Station,

Units 1, 2,

and 3, facility.

At the conclusion of

the inspection,

the findings were discussed

with those

members of your staff

identified in the enclosed report.

Areas

examined during the inspection

are identified in the report

and included.

a review of your steam generator history and material condition and inservice

inspection.

Within these

areas,

the inspection consisted of selective

examinations of procedures

and representative

records,

interviews with

personnel,

and observation of activities in progress.

The results of this

inspection

are documented

on page

2, in the enclosed report.

I

Overall, the results of the inspection indicate that you have

implemented

a-

comprehensive

eddy current examination

program for detection of degradation

in

steam generator tubing.

Other programmatic

actions

have also

been taken which

should

be beneficial

in terms of reducing the incidence of further tube

degradation.

Of particular

note

has

been the reduction in hot leg

temperature,

performance of chemical

cleaning of steam generators,

and the

implementation of various secondary

water chemistry initiatives.

While it is

acknowledged that improvements

in the chemistry information management

record

system are planned, it should

be noted that the existing system impairs the

assessment

of historical chemistry performance

and its possible contribution

to steam generator

tube degradation.

One other subject that we believe

warrants

management

attention is the identification of the contributory

reasons for variations in steam generator

tube degradation

rate

between the

three units and within each unit.
