This r efers to the inspection
conducted
by K. Johnston of this office, and the
inspectors identified in the enclosed
report
on February
15 through Narch 28,
1994.
The inspection
included
a review of activities authorized for your
Palo
Verde facility.
At the conclusion of the inspection,
the findings were
discussed
with those
members
of your staff identified in the enclosed report.
Areas
examined during the inspection
are identified in the report.
Within
these
areas,
the inspection consisted of selective
examinations of procedures
and representative
records,
interviews with personnel,
and observation of
activities, in progress.
This inspection identified several
areas
demonstrating
a lack of thorough
engineering evaluation
and followup of plant problems.
In one case,
a
4-year-old modification to essential
chiller instrumentation
was not reflected
in plant annunciator
response
procedures
and in the Final Safety Analysis
Report,
and your engineering staff still considered
the modification
temporary.
In another
case,
even though plant procedures
and personnel
recognized
the potential for a water
hammer event in Unit 1's
blowdown system during a manipulation- of the system's
containment isolation
valves,
on February
14,
1994, operators
caused
a water hamet
which damaged
a
pipe support
on this line.
The inspection
found that your engineering staff
had not fully evaluated
the impact of previous events
and
had not.followed
through
on
a modification to prevent the water
hammer.
We encourage
you to
review these
and the other examples
discussed
in the enclosed report
and to
determine
where the opportunities
were missed to correct these
problems.
-The inspection also found that the high intensity training for licensed
operators
was well implemented.
The program featured challenging scenarios,
in-depth critiques,
and management
involvement.
Operators
demo'nstrated
good
command
and control
and excellent communications.
We recognize that this

effort demonstrates
considerable
management
commitment
and initiative and
anticipate its benefits in improved operations
performance.
The inspection identified one unresolved
item concerning the necessity of
declaring
a Notification of Unusual
Event during
a Unit 2 event where reactor
coolant system inventory inadvertently was drained to the refueling water
tank.
This matter is discussed
in Section 3.4 of the enclosed
inspection
report.
In accordance
with 10 CFR Part 2.790 of the NRC's "Rules of Practice,"
a copy
of this letter and its enclosures will be placed in the
NRC Public Document
Room.
