This refers to the inspection
conducted
by K. Johnston,
H. Freeman,
J. Kramer,
and A. MacDougall of this office, and T. Alley, of the Department of Energy,
on December
7,
1993,
through January
10,
1994.
The inspection
included
a
review of activities auth'orized for your Palo Verde facility; At the
conclusion of the inspection,
the findings were discussed
with those
members
of your staff identifi'ed in the enclosed report.
Areas examined during the inspection are identified in the report.
Within
these
areas,
the inspection consisted of selective
examinations of procedures
and representative
records,
interviews with personnel,
and observation of
activities in progress.
No violations or deviations
were identified within the scope of this
inspection.
However, the inspection identified an issue that appears
similar
to the root cause of,a recent violation involving the failure to accomplish
surveillance
actions within the Technical Specifications
time requirements
following the removal of an emergency
diesel
generator
from service.
The
inspectors
noted that
a surveillance
test which verified the operability of
associated
equipment
was not performed
when
a tr ain of the essential
chilled
water system in Unit
1 was taken out of service
on December
10,
1993 for
planned corrective maintenance.
Although the essential
chilled water system
was restored within
1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br /> as required
by Technical
Speci fications, this
restoration
took 57 minutes
and action had not been initiated within this time
to perform the required verifications.
We are concerned that the shift
supervisor took an unnecessary
risk in not performing the verification earlier
and that there
was not sufficient management
guidance provided concerning
when
.
this surveillance verification must be initiated.
In accordance
with 10 CFR Part 2.790 of the NRC's "Rules of Practice,"
a copy
of this letter and its enclosures will be placed in the
NRC Public Document
Room.
