Initial Reactor'perator, Senior Reactor Operator, and Senior Operator Limited to Fuel Handling Licensing Examinations at Palo Verde Nuclear Generating Station, Units 1, 2 and 3
During the period September 13-21, 1993, the NRC administered initial operator and senior operator licensing examinations to employees of your company who had applied for licenses to operate your Palo Verde Nuclear Generating Station,.Units 1, 2, and 3.
In addition, the NRC administered a retake operating examination to a Senior Operator Limited to Fuel Handling (LSRO) on September 21, 1993.
As a result of these examinations two out of five Senior Reactor Operator (SRO) and nine out of twelve Reactor Operator (RO) applicants met the minimum qualification requirements for licensing.
In addition, the LSRO applicant met the minimum qualification requirements for licensing.
During this examination, operator performance deficiencies resulting in failures were more numerous than expected.
The three Job Performance Heasure (JPH) failures appear to have been caused by inattention to detail and lack of knowledge.
JPH training requires additional attention.
The four simulator failures demonstrate that some candidates had difficulty diagnosing events and following procedures.
The candidates'ifficulty was especially evident when there was more than one malfunction or significant event being managed at the same time.
The test results and our examiners'bservations appear to confirm our previous observations from the August 1993 training inspection.
Consequently, continued emphasis on JPH training and additional training on Emergency Operating Procedures (EOPs) in the simulator appear warranted to improve operator and candidate performance.
Please be-prepared to discuss your evaluation of these two issues and appropriate plans for improvement at our November 15,
