The NRC has received information about activities at your Palo Verde Nuclear Generating Station.
The details of this information are included in the enclosure to this letter.
We request that you review this information and provide us a response within 60 days.
The enclosed information also includes documentation of four trends that reportedly have not been identified and resolved by your corrective action program.
We also request that you provide an assessment of the trends in your response to the concerns.
If the enclosed information is substantiated, your response should include any proposed corrective actions you believe are appropriate.
The enclosure to this letter should be controlled and distribution limited to personnel with a need to know.
The enclosure to this letter is being withheld from public disclosure in accordance with the provisions of 10 CFR 2.790.
If your response contains any privacy or privileged information that you believe should be withheld from public disclosure, please provide that information in an appropriately marked enclosure to your letter.
In addition, if you provide any information which you believe is proprietary, please provide the affidavit required by 10 CFR 2.790.
