Subject:

Palo Verde Nuclear Generating Station, Unit 2 Docket No. STN 50-529 Steam Generator Tube Rupture File: 93-056-026 On March 14, 1993, Palo Verde Nuclear Generating Station (PVNGS), Unit 2 experienced a steam generator tube rupture. The referenced letter transmitted Licensee Event Report 93-001-00 which describes the event.

Arizona Public Service Company (APS) formed a task force to perform a comprehensive root cause'f failure analysis and to develop an extensive inspection plan for both Unit 2 steam generators.

This task force is composed of APS personnel and other industry personnel with vast experience in steam generator technology. Since the March 14, 1993 tube rupture, there have been numerous discussions with the NRC regarding the status of the root cause analysis and the progress of the inspection plan.

The purpose of this letter is to advise the NRC that APS will continue its updates on the progress of the inspections of the Unit 2 steam generators and will inform the NRC upon completion of each phase of the inspection plan.

APS will also continue to update the NRC regarding actions being taken to determine the root cause of the tube rupture, as well as observed tube degradation, and will meet with the NRC as necessary to discuss these actions and the results.

Nuclear Regulatory Commission ATIN: Document Control Desk Steam Generator Tube Rupture Page 2 APS will provide for NRC review an analysis which contains, as a minimum, the results of APS'nvestigation into the root cause of the steam generator tube rupture, corrective actions, and the basis for restart of PVNGS Unit 2.

APS will meet with the NRC to discuss this analysis following its submittal. PVNGS Unit 2 willnot be restarted until these matters have been addressed and startup has been discussed with the NRC.
