ATTACHMENT Re: St. Lucie Unit 1 Docket No. 50-335 Ins ection Re ort 335/85-29 FINDING 10 CFR 50, Appendix B, Criterion V, as implemented by FPRL Topical Report (FPLTQAR 1-76A) Section TQR 5.0, Instructions, Procedures, and Drawings, Revision 5 requires that procedures, instructions, or drawings shall include appropriate quantitative and qualitative acceptance criteria for determining that activities have been satisfactorily accomplished.

Contrary to the above, as of November 15, 1985, adequate acceptance criteria were not included in General Maintenance Procedure, GMP 1-M-0015, to ensure that the upper guide support lifting rig lifting bolts were fully engaged into the upper guide 'structure (UGS) flange during removal of the UGS internal. This resulted in a failure of one of the three lifting bolts causing the 05 ton UGS to be in a tilted position over the reactor vessel and fuel assemblies.

RESPONSE

l. FPL concurs with the finding.
2. The reason for this finding was procedural inadequacy, in that the procedure for the removal of the upper guide structure internals did not contain acceptance criteria for full thread engagement of the VGS lift bolts.
3. Procedure 1-M-0015 will be revised to provide acceptance criteria for full thread engagement of the UGS lift bolts.

Applicable procedures will 'be reviewed and revised to ensure inclusion of acceptance criteria for thread engagement.

5. Full compliance will be achieved by March 31, 1986.
