NOTICE OF VIOLATION Arizona Public Service Company Palo Verde Units 1, 2,
3 Docket Nos.
50-528, 50-529, 50-530 License Nos.
NPF-41, NPF-51, NPF-74 During an NRC investigation conducted on November 5 through December 19,
- 1992, a violation of NRC requirements was identified.
In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions,"
10 CFR Part 2, Appendix C, the violation is listed below:
10 CFR 50.9(a) states in part:
" Information provided to the Commission...
by a licensee... shall be complete and accurate in all material respects."
Contrary to the above, during a meeting on Hay 16, 1990, licensee officials provided information to the NRC regarding the failure rate of emergency lighting units required by NRC regulation which were not complete and accurate in all. material respects.
In particular, emergency light failure rates were actually substantially higher than the 5% to 10% value provided during the meeting.
This is a Severity Level IV violation (Supplement I).
