PROPOSED IMPOSITION OP CIVIL PENALTY 0100,000 This is to inform the Commission that a Notice of Violation and

.Proposed Imposition of Civil Penalty in the amount of $ 100,000 will be issued on or about August 13, 1992 to Arizona Public Service.

This action is based on a number of violations which collectively indicate a significant problem with work control at the plant and they have therefore been classified as a Severity

-Level III problem.

The violations resulted in three events which were of concern.

In the -first, inadequate work control resulted in the failure of a reactor trip breaker to open during testing and the subsequent fai'lure to preserve the as-found condition of the failed breaker.

The second involved the installation of a check valve, required to provide containment isolation, in reverse position.

The third involved improper actions on the part of an electrician which resulted in the total loss of control room annunciators for more than a day.

The base civil penalty of $50,000 has been escalated 50 percent for poor prior performance and an additional 50 percent for prior notice.

It should be noted that the licensee has not,been specifically informed of the enforcement action.

The schedule of issuance and notification is:

Mailing of Notice August 13, 1992 Telephone Notification of Licensee August 13, 1992 The State of Arizona will be notified.

The licensee has thirty days from the date of the Notice in which to respond.

Following NRC evaluation of the response, the civil penalty may be remitted, mitigated, or imposed by Order.
