SUBJECT:

NRC TEAM INSPECTION OF EMERGENCY OPERATING PROCEDURES,

REPORT NOS. 50-528/92-12, 50-529/92-12 AND 50-530/92-12

This report transmits the findings of the NRC team inspection conducted at the Palo Verde

Nuclear Generating Station from April6 through April 10, 1992. The purpose of the inspection

was to evaluate the new revision of the Palo Verde Emergency Operating Procedures

(EOPs).

At the conclusion of the inspection, the preliminary findings were discussed with you and those

members of your staff identified in the enclosed report.

, Arizona Public Service has devoted extensive resources

to revise and improve the Emergency

Operating Procedures

used at Palo Verde. The team determined that the operators would be able

to utilize the new EOPs to mitigate emergency conditions.

The new procedures are scheduled

for implementation later this year.

You have made substantial progress in correcting problems

previously identified through 'an intensive upgrade

program

involving a multi-disciplined

approach and development of a comprehensive writers'uide.

While we see clear evidence., of increased commitment to the procedure development process;

the team also identified several weaknesses

requiring your attention, prior to implementation of

the new procedures.

We are concerned

that many of the problems identified during this

inspection had also been identified during the last EOP inspection.

We consider that your staff

would have identified most of the issues ifa more aggressive verification and validation program

was in place.

Significant. problems identified include:

(1) an inadequate isolation strategy for

a loss of coolant accident outside of containment; (2) conflicting information among the EOPs

when confronted with a steam generator tube rupture in one steam generator coincident with an

excess steam demand event in the other steam generator; (3) incomplete information in the EOPs

when adverse

containment conditions exist; (4) inconsistent application of standard

post trip

actions with regard to verification of safety functions; (5) conflicting direction given to the

operators*with respect to procedural adherence

when in the EOPs; and (6) weaknesses

in the

quality organization's

involvement

as part of the EOP development

process.

It is our

understanding

that the first five items identified above will be corrected

prior to. your

implementation of the EOPs.

In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of this letter and

its enclosure willbe placed in the NRC Public Document Room.

with you.
