Palo Verde Nuclear Generating Station (PVNGS)
Units 1, 2, and 3 Docket Nos. 50-528/529/530 Revised Completion Date For Corrective Action File: 92-070-026 Arizona Public Service Company (APS) responded, in the referenced letter, to the notice of violations issued in EA 91-182.
Attachment 1 to the referenced letter described the immediate and longer term actions that are being taken on a site wide basis to eliminate the root causes which led to the violations.
APS committed, in Attachment 1 to the referenced letter, to complete an organizational and programmatic evaluation of personnel performance at PVNGS by May 1992. Due to APS'nvestigation activities associated with the water hammer related to the March 23, 1992, Unit 2 reactor trip, the March 31, 1992, surveillance test failure of a Unit 3 reactor trip breaker, and the Unit 3 ALERT emergency classification from May 4, 1992, through May 6, 1992, this date will not be met.
The first phase of the organizational and pro'grammatic assessment, which was completed in March 1992, concluded that detailed organizational and programmatic assessments of the Operations, Maintenance and Technical Support Departments and their key work processes be conducted.
To date, the evaluation of the Operations Department has been completed, the evaluation of the Maintenance Department is expected to be completed by July 31, 1992, and the evaluation of the Technical Support Department is expected to be completed by August 31, 1992.
