Essential Cooling Water Heat Exchanger Plugged Tubes File: 92-019-026 This letter is being submitted to provide follow-up information regarding the number of tubes plugged during repairs to the Train A Essential Cooling Water System (ECWS) heat exchanger for which the referenced TWOC was submitted.
ln the referenced iWOC, it was indicated that as of 9:00 a.m. MST on March 15, 1992, work remaining to be completed included plugging seventy-one tubes which had indication of potential defects when eddy current inspected.
This status was based upon best available information at that time.
Subsequent final review of the work performed and compilation of the data shows that eighty-eight tubes were plugged.
This is less than the maximum number of tubes (128) which can be plugged per the manufacturer's recommendations.
Of the eighty-eight tubes plugged, fourteen were plugged due to visual confirmation of leakage, six were plugged due to potential damage incurred when one tube was removed for root cause analysis, and sixty-eight tubes were plugged due to indications of potential defects.
Eddy current inspection was performed on 101 tubes along their full length and on all tubes up to their first support plate.
The confirmed leaks and tube defects were found in the tube area up to the first support plate.
During eddy current inspection, sixty-eight tubes were found to have potential defects and were plugged at management's direction prior to engineering final analysis of the eddy current results.
This decision was made to minimize the out-of-service time for the heat exchanger while remaining within the design of the heat exchanger as stated in the manufacturer's technical manual.
