'I Arizona Public Service Company (APS) continues to demonstrate that the Fitness-for-Duty Program is successful towards achieving a drug-free work place.

During the 1991 calendar year, APS performed 4,787 random drug and alcohol tests. This yields a testing rate of 128% for the average population with unescorted access.

The incidence of confirmed positive random drug and alcohol tests during calendar year 1991 (0.2%) is less than that in.1990 (0.3%).

Anal sis of Pro ram Results The confirmed positive drug test results by type of test and type of drug involved for this reporting period are summarized in Tables 1 and 2, and are described as follows:

Pre-Badge Testing:

A total of 1,488 individuals were pre-badge tested.

The positive test rate for this reporting period was 0,3%.

All positive results were from contractor personnel.

This is significantly lower than the last reporting peiiod (1.1%); and lower than the 1990 time period from July 1990 through December 1990 (0.7%).

One conclusion drawn from this decrease is that individuals who are req'uesting unescorted access are more aware of our Fitness-for-Duty Program requirements.

  • One individual tested positive for two drugs (marijuana and alcohol).

Comments:

APS tested 47 individuals for cause during this reporting period. The causes included the odor of alcohol on individuals, behavioral observations, allegations, and suspected adulteration of urine specimens.

In 4 of the cases where the odor of alcohol was detected, the tests were positive. One person was an APS employee and the other 3 were contractors.

The 3 positive drug tests were the result of suspected adulteration of urine specimens.

Of these, 1 was an APS employee and 2 were contractors.

Five of the individuals who tested positive were on the day shift and 2 were on the night shift.

Random Testing:

APS tested 2,074 individuals selected randomly for drug and alcohol tests during this reporting period. This yields an annualized testing rate of 112%.

Of the 5 individuals testing positive, 2 were APS employees, and 3 were contractor personnel.

Four individuals tested positive on their first random test in 1991, and 1 tested positive on his second random drug test. Four out of 5 individuals were day shift employees, and 1 was a night shift employee.

Post-Accident Testing:

Comments:

Five persons were tested during this reporting period. Alltest results were negative.

Comments:

Under the APS follow-up testing program,, a total of 93 tests were

, conducted during this reporting period. Alltest results were negative.

At the end of this reporting period, 15 individuals were under the follow-up testing program based on the results of the "suitable inquiry," and 2 were in the program as a result of a positive alcohol test at APS: Of those 17 individuals, 6 were APS employees and 11 were contractor personnel.

Other Testing:

. Failure to Cooperate

  • One individual left the site without appearing for random testing.

That refusal to cooperate was deemed to be a positive test.

Comments:

In addition, 16 tests were administered to a contractor employee in cooperation with the Arizona Treatment Assessment Screening Center program.

All of those tests were negative.

In each case of a confirmed positive drug test result, the individual's site access was revoked.

APS employees testing positive for illegal drugs were discharged from employment.

APS employees who had a single occurrence of a positive test for alcohol received a disciplinary suspension, evaluation, and treatment prior to reinstatement of unescorted access.

In addition, those individuals were placed on a three-year follow-up testing program.

Contractor personnel who tested positive for alcohol had their site access revoked.

Pro ram Review A Quality Assurance audit was conducted in November 1991.

A consulting group was utilized to provide technical guidance during the audit, Results from the audit indicated that APS has a comprehensive Fitness-for-Duty Program which meets the requirements of 10 CFR 26. Many of the recommendations that were made during the audit are being incorporated into the program.

Three performance test specimens (certified by the supplier to be positive) were reported negative by the APS primary Department of Health and Human Services (DHHS) certified laboratory.

Two of the occurrences were the result of a urine matrix structure using a reagent designed to have a sensitivity to a wide range of drugs.

The standards utilized by the laboratory were appropriate for this particular drug.

However upon closer investigation of the standards, it was determined that the use of another standard was more sensitive.

Therefore, changes were made in -the standards utilize'd by the laboratory.

p 1 61 -04631 -WFC/NLT February 25, 1992 In the other occurrence it was determined that the certified blind specimens deteriorated during the shipping and handling process.

The type of drugs used to spike the specimens have a tendency to be unstable and may contribute to the deterioration of the specimen.

The Medical Review Officer reviewed APS'rocess for preparation of all specimens and determined the process acceptable.

During the reporting period, APS selected Poisonlab, Inc. of San Diego as the new

'ackup DHHS certified laboratory.

The change in backup laboratory.was made to improve the level of service and performance.

Three individuals appealed their test determinations during this reporting period.

Two were APS employees and one was a contractor.

The appeals board reviewed the relevant material and reaffirmed the original determination.

Summa of Pro ram Chan es The following are changes that have been made to the Fitness-for-Duty Program in an effort to improve overall effectiveness of the program:

In an effort to provide readily accessible guidance on the call-out requirements set forth in the Fitness-for-Duty Program, an informational card which can be affixed to a badge was sent to.all supervisors.

The card provides instructions to assist supervisors in the event it is necessary to call an individual out for unscheduled work or for an emergency situation.

A collection facility became operational inside the protected area during this reporting period.

The purpose of the facility is to reduce the impact on licensed operator shift manning and overall site productivity.

'I During this reporting period, APS began using two new chemical analyzers (Syva ETS+ model) to perform on-site drug screening.

The utilization of this equipment was identified as a strength during our November audit. Performance results from this equipment is favorable.

4 A random drug test "Notification Checklist" was developed and disseminated to supervisors as a tool to assist them in completing the necessary notification steps.

This effort is part of APS'ommitment to provide ongoing assistance and information to supervisors.

APS has reviewed and revised some of its procedures and training material to incorporate changes and program requirements in accordance with 10 CFR 55.

A total of 8,938 tests has been conducted during the 24-month period ending December 1991.

Summary of Event:

On November 10, 1991, at approximately 2200 hours0.0255 days <br />0.611 hours <br />0.00364 weeks <br />8.371e-4 months <br />, a Firewatch Foreman found a small bag (approximately 2 inches by 1 1/2 inches) containing a tablespoon of seeds and cuttings on the floor of the Firewatch Foreman's office.

The office is located in the protected area, at the 140 foot level ofthe auxiliary building near the access to the turbine building breezeway.

An investigation was conducted by APS Security to determine the source and makeup of the substance.

The substance was tested and determined to be marijuana.

The investigation indicated the small packet of marijuana was apparently slipped under the locked door to the Firewatch office during the swing shift prior to 2200 hours0.0255 days <br />0.611 hours <br />0.00364 weeks <br />8.371e-4 months <br />.

No individuals could be associated with the substance.

APS uses different cut-off levels than the NRC for several drugs.

If the NRC screening cut-off level (100 ng/ml) for marijuana had been used in the testing program instead of the APS cut-off level (20 ng/ml), 8 individuals would not have tested positive for marijuana.

One individual tested positive for two substances, marijuana and alcohol as a result of random testing.
