This refers to the inspection

conducted

by Messrs.

D. Coe,

F. Ringwald,

J. Sloan,

and

W. Ang of this office on November'10

through

December

17,

1991.

The inspection

included

a review of activities authorized for your Palo Verde

facility.

At the conclusion of the inspection, the'indings

were discussed

with,those

members of your staff identified in the enclosed report.

Areas

examined during the inspection

are identified in the report.

Within

these

areas,

the inspection consisted

of selective

examinations

of procedures

and representative

records,

interviews with personnel,

and observations

of

activities in progress.

Based

on the results of this inspection,

certain of your activities

appeared

to be in violation of

NRC requirements,

as specified in the enclosed

Notice of

Violation.

Each of the three

examples of failure to follow procedures

involves

a cognitive error

on the part of

a licensed operator.

Other

examples

of failure to adhere to procedural

requirements

were discussed

with

you at

an Enforcement

Conference

on December

18,

1991.

In your response

to

the enclosed

Notice, please

discuss

your actions to communicate

your

expectations

to plant personnel.

You are required to respond to this letter

and should follow the instructions

specified in the enclosed

Notice when preparing

your response,

In your

you should

document the specific actions

taken

and

any additional

actions

you plan to prevent recurrence.
