This refers to the enforcement
conference
and management
meeting held with
Arizona Public Service
Company at the Region
V office on December
18,
1991.
Subjects
discussed
during the meeting are described
in the enclosed
Neeting
Report.
The purpose of the enforcement
conference
was to discuss
the circumstances
and
apparent violations associated
with (1) conducting core alterations without
the supervision of a licensed
senior reactpr operator
and without'establishing
direct communications
between the control
room and the refueling station
and
(2) failing to adhere
to procedural
requirements for maintaini'ng required
refueling pool level, obtaining boron samples
and responding to a control
room
alarm indication.
Also addressed
were the findings of the Augmented
Inspection
Team as detai'led in report
No. 50-530/91-47,
regarding
the mobile
crane event of November
15,
1991.
These events
are indicative of a lack of formality in implementing'our
management
program for the oversight
and control of activities. It appears
in
these
instances
that your practices
did not ensure that the designated
personnel
could adequately
maintain control of the associated
work.
During the management
meeting that followed the enforcement
conference,
you
discussed
the status of your steam generator
loose pa'rt monitoring and fire
protection
improvement programs.
Summary of these discussions
are also
included in'the enclosed report.
In accordance
with 10 CFR 2.790(a),
a copy of this letter and the enclosed
report will be placed in the
NRC Public Document
Room.
