161-04548-WFC/ZMQ December 30, 1991 ATTACHMENT CLOSURE OF NRC BULLETIN 88-10 AND SUPPLEMENT 1 Arizona Public Service Company (APS) would quarantine the safety-related non-traceable molded-case circuit breakers (MCCBs) for the requested one-year period.
STATUS:
The safety-related non-traceable MCCBs were quarantined for the requested one-year period and were subsequently disposed of.
APS procurement and dedication practices for MCCBs were being revised to meet or exceed the criteria delineated in Bulletin 88-10.
STATUS'he Item Procurement Specification Requirements Procedure was revised to include guidelines from Bulletin 88-10'.
These guidelines state that MCCBs installed in safety-related applications should be manufactured by and procured'rom a Circuit Breaker Manufacturer (CBM) under a 10 CFR 50 Appendix B,program or procured from a
CBM or other supplier.with verifi:able traceability to the CBM. It further states that NRC Bulletin 88-10 Supplement 1 defines verifiable traceability as a shipping notice or packing slip from the CBM to the,supplier.
A CBM's Certificate of Compliance to the supplier or a Telephone Record'heet i;s not adequate documentation.
- Also, the Commercial Grade Dedication Requirements, Procedure was revised to state that Commercial Grade circuit breakers shall not be purchased nor shall. Palo Verde Nuclear Generating Station (PVNGS) nonquality-related circuit breaker stock be upgraded by the use of Commercial'rade evaluation.
OPEN ITEM' Two circuit breakers were issued for use in a safety-related application in Unit 1.
As a result, a
PVNGS work order has been issued to replace the.installed circuit breaker during the current PVNGS Unit 1 outage.
STATUS'PS cannot locate, work documents issuing the breakers for a specifi'c application.
However, the Quality Receiving Checklist provided verification that the breakers were intended for a specific application in Unit l. It is presumed that one of these breakers was installed'n the specific application in Unit 1 which was removed and replaced in March 1990, during the refuel'ing outage.
The other breaker most likely failed to meet the acceptance test requirements, prior to its

i 161-04548-WFC/JMQ December 30, 1991 intended use and was disposed of. It is considered'nlikely that one of these breakers purchased for a specific application could have been issued for use in another application.
APS identified the other safety applications for these circuit breakers.
The work order history for those applications was reviewed and there was no documentation that the non-tracable circuit breaker was utilized in

those applications.
However, in the unlikely event that it could have been installed in another application, the following justifies the acceptability of the circuit breaker and should eliminate any concern:
The circuit breaker was purchased from a local distributor, J'.
E.
Redmond through a
regional distributor, MAG-TROL.
Although both vendors produced'raceability and procurement records for only selling new and factory originated circuit
- breakers, their records were not kept for more than five years.
Due to the procurement date of the circuit breakers (August, 1983) these circuit breakers were deemed non-traceable, since no traceability documentation was found.
NRC Bulletin 88-10 1'imited the scope of the MCCBs to those that were purchased between August 1, 1983 and August 1, 1988.
The purchase order for the Redmond circuit breakers is dated.
August 29,
- 1983, which is less than a month after the start date of the Bulletin.
When considering factors such as warehouse storage and distribution lead time, the manufacture date of the Redmond procured circuit breakers would have probably been prior to the NRC chosen, start date of August 1',
1983 for the scope of the Bulletin.
This should minimize the possibility of these breakers being suspect.
Each system at PVNGS is designed redundantly to assure the system safety function can be accomplished, assuming a single failure.
As a result, the possibility of a
common mode failure does not exist in the subject case since only a singl'e component is involved.
Preoperational, Maintenance, and Surveillance Tests have been performed on PVNGS safety-related breaker applications to ensure that the breaker meets its functional characteristic requirements as specified by the manufacturer.
Based on the above,, all actions to NRC Bulletin 88-10 and Supplement 1 have been completed.
