PALO VERDE UNITS I, 2,

AND 3

This refers to the Augmented Inspection

Team (AIT) inspection

conducted

by

Messrs.

L. Miller, D. Acker, J. Sloan,

and

N. Trehan during the period of

November 16-19,

1991.

This inspection

examined

the partial loss of offsite

power event at Unit 3 which occurred

on November

15,

1991,

and other

activities authorized

by

NRC License

Nos.

NPF-41,

51,

and 74.

At the

conclusion of the inspection,

the inspectors

met with Messrs.

J. Bailey,

i

. J. Levine,

and other members of your staff on November 20,

1991, to discuss

the findings of the inspection.

Areas

examined during this inspection

are described

in the enclosed

inspection

report.

Within these

areas,

the inspection consisted

of selective

examinations

of procedures

and representative

records,

interviews with personnel,

and

observations

by the inspectors.

Our inspection

concluded that this event occurred primarily due to:

(1) the

crane

being left unattended;

(2) failure of involved personnel

to electrically

ground the mobile crane

as required

by your accident prevention'anual;

and

(3) poor initial communication

between

the site of the event

and the control

room.

Many other factors contributed to the event.

Among these

were

inadequate

procedures for the safe operation of mobil'e cranes

near energized

equipment,

weak work planning

and control practices,

and excessive

work hours

for some of the personnel

working on this job.

Our inspection further concluded that your staff 's review of the lessons

learned for the Vogtle and Diablo Canyon loss of power events

had not been

effective in preventing this event.

Your responsible

line departments

had

reviewed their existing controls for operation of equipment

near energized

switchyards

and transmission

lines,

but had not proposed sufficient revisions

to these controls to ensure

the safe conduct of switchyard activities.

It is particularly discouraging to note that this event occurred after

senior

NRC Region

V management

discussed

the Vogtle and Diablo Canyon events

with senior

APS management,

and

we were assured

that Palo Verde

had reviewed

and learned

from both of these

events.

Moreover,

as this report indicates,

in interviews with personnel directly involved in the event,

none of them

were aware of the Vogtle or Diablo Canyon events.
