91-005-419.06; 91-056-026 The 1991 NRC evaluation of the requalification program and licensed personnel was conducted at PVNGS during the weeks of September 9
and September 16, 1991.
Arizona Public Service Company (APS) is providing Enclosure A in accordance with the guidance of Section C.l.a.(7) of Examiner Standard ES-601, "ADMINISTRATION OF NRC REQUALIFICATION PROGRAM EVALUATIONS," (NUREG-1021, Revision 6,
dated June 1, 1990).
Enclosure A furnishes an overall summary of the 1991 PVNGS NRC Requalification Examination scores and the examination scores for the Written Examination, as well as the results of the evaluations of Job Performance Measures and Simulator Scenarios, for each of the examinees.
The results reported in this enclosure represent the findings of the PVNGS Evaluators.
APS requests that the information in Enclosure A (so designated) be considered confidential and withheld from public disclosure pursuant to 10 CFR 2.790(a)(6) and 10 CFR 9.17(a)(6).
Enclosure B provides an evaluation of the validity of the Category A examination test items.
This enclosure includes a specific evaluation of Test Item Number A273 (from Category A) that was deleted from the examination by the PVNGS Examination Team during the post-examination review.
Enclosure C provides the Pre-Examination and Pos't-Examination Security Agreements executed by the 19 PVNGS personnel allowed access to the NRC Requalification Examination material prior to and/or during the examination.
Please note that Mr. D.
C. Rogers ceased employment at PVNGS prior to the examination and signed an agreement at that time.
Subsequent to the completion of the examination, although he was no longer employed at
- PVNGS, he signed a
Post-Examination agreement to confirm that he did not release any information concerning the examination during the intervening period.

S. Nuclear Regulatory Commission Requalification Program Evaluation Page Two A reexamination of two applicants from the June 1991 operator licensing examination was conducted during the week of September 23, 1991.
The two PVNGS members of the reexamination team (Messrs.
Aho and Shouse) extended their respective security agreements (that were executed for the requalification examination) through this reexamination period.
