REPORT
NO. 50-528/529/530/91-26
This refers to the inspection
conducted
by Messrs.
D.
Coe, J.
Ringwald, J.
Sloan,
and J. Melfi of this otfice on June
16 through July 27,
1991.
The
inspection included
a review of activities authorized for your Palo Verde
facility.
At the conclusion of the inspection,
the findings were discussed
with those
members of your staff identified in the enclosed report.
Areas
examined during the inspection
are identified in the report.
Within
these
areas,
the inspection consisted of selective
examinations of procedures
and representative
records,
interviews with personnel,
and observation of
activities in progress.
Based
on the results of this inspection, certain of your activities appeared
to be in violation of NRC requirements,
as specified in the enclosed
Notice of
Violation.
After careful consideration
of the circumstances
involving the
incorrect setting of four core protection calculator constants,
we have
decided to issue
a violation, although it meets
the criteria of a non-cited
violation.
This decision
was based
on the significance of the core protection
calculators,
the fact that independent verification of the values did not
identify the errors,
and the past errors
and deficiencies
noted with core
monitoring computer
systems.
This violation and other examples of insufficient attention to detail
noted
herein indicate
a continuing need to emphasize
better adherence
to procedural
and administrative controls.
This was discussed
at our recent
management
meeting with Mr. Levine and others of your staff.
You are required to respond to this letter and should follow the instructions
specified in the enclosed
Notice when preparinq your response.
In your
response,
you should
document the specific actions
taken
and any additional
actions you plan to prevent recurrence.
After reviewing your response
to this
Notice, including your proposed corrective actions
and the results of future

the
NRC will determine whether further
NRC enforcement
action is
necessary
to ensure
compliance with NRC regulatory requirements.
In accordance
