91-005-419.06 In accordance with the requirements of 10 CFR 55.35, Arizona Public Service Company (APS) hereby requests that a re-examination be administered to the two individuals identified in the enclosed applications.
The enclosures provide an original and two copies of NRC Form 398, "PERSONAL QUALIFICATION STATEMENTS-LICENSEE," for the two applicants for re-examination.
Requests for waivers are provided within the applications.
Based on a discussion between Mr. Edwin Firth, General
- Manager, PVNGS Nuclear
- Training, and Mr. Gary Johnston, NRC Region V, the re-examinations could be conducted during the upcoming evaluation of the requalification program scheduled at PVNGS during the weeks of September 9 to September 27, 1991.
The NRC Form 396, "CERTIFICATION OF MEDICAL EXAMINATION BY FACILITY LICENSEE,"
submitted for each of the applicants as an enclosure to the referenced letter is still valid (medical examination completed within the past six months); however, these will expire prior to the re-examination.
These individuals are currently being scheduled for new medical examinations which will be completed, and the results forwarded to you prior to the re-examination, as discussed with Mr.
Johnston.
Submittal of the new NRC 396 Forms will complete the re-application.
