Areas
examined during this inspection
are described
in the enclosed
inspection
report.
Within these
areas,
the inspection consisted of selective
examinations of procedures
and representative
records,
interviews with
personnel,
and observations
by the inspectors.
No violations or deviations
were identified during this inspection.
However,
this report discusses
several
instances
in which management
or supervisory
personnel
were either not aware of conditions
under their cognizance
or did
not follow through
on identified issues,
particularly in requesting
engineering
support for decisions
made in the field.
An example of this was
the decision to cover auxiliary building floor drains with plastic without
first determining the impact this would have
on the flooding analysis,
and the
subsequent
replacement
of this plastic with a soluble plastic without
evaluating the compatibility of this material with the drain system.
This
event
and others described
in this inspection report, involving motor operated
valve grease
conditions,
Agastat relay setpoint stability,
and Unit 3 foreign
material exclusion control, reflect
a need for personnel
to develop
a more
questioning attitude
when making decisions
or touring the facility.
In accordance
with 10 CFR Part 2.790(a),
a copy of this letter and the
enclosure will be placed in the
NRC Public Document
Room.
