ESSENTIAL LIGHTING Me understand that you recently identified conditions in certain locations in the Palo Verde units where the use of essential lighting only (without the normal or battery powered lights) would not be sufficient for access,

egress, and performance of required safe shutdown operations.

This condition was described in Material Nonconformance Report (MNCR) 91-gB-9005.

Me further understand that a Conditional Relief Request was signed on April'5, 1991, which noted that there had been no final disposition to tIre MNCR.

The Conditional Relief Request states that the essential lighting in question was unavailable and that compensatory measures had been taken.

These compensatory measures included the issuance of letters to each Palo Verde unit, (I) noting the possible deficiencies, (2) requesting the issuance of Night Orders describinq the situation, and (3) advising that flashlights would be needed in certain s>tuations and that procedures should be changed to require certain manual actions to allow the emergency lighting system to provide the necessary lighting.

Me also understand that the reportability of this matter to the NRC is currently under review.

Please provide us with the results of your evaluation of this matter within 30 days of the date of this letter.

Your response should include:

(1) your corrective actions; (2) your assessment of whether regulatory requirements were satisfied despite the reported condition; (3) your assessment of whether this matter is reportable to the NRC in accordance with 10 CFR 50.72 or 50.73; and (4) a description of the evaluation of these or similar deficiencies when they were identified in walkdowns in 1989.
