AUDIT OF PALO VERDE NUCLEAR GENERATING STATION (PVNGS)

ON BULLETIN 88-11 ISSUES REGARDING SURGE LINE THERMAL STRATIfICATION The staff of the Mechanical Engineering Branch of the Nuclear Regulatory Commission and its consultant, the Brookhaven National Laboratory, conducted an audit of the Palo Verde Nuclear Generating Station (PVNGS) on February 20-21, 1991.

The purpose was to review the implementation and results of your program to addressBulletin 88-11 issues regarding pressurizer surge line thermal stratification.

The Bulletin requested all PWR plants to conduct an inspection of the surge line and its supports for possible distress or damage, and to perform reanalysis to confirm that the surge line meets the applicable code requirements for the licensed life of the plant.

During the audit, technical details and status of your program were discussed.

The team also reviewed several selected documents including inspection reports, operating logs and procedures, calculation

packages, monitoring data, and plant

- drawings.

A detailed audit report which delineates our efforts, findings and conclusions is enclosed.

Inspection of the surge line in all three units has been completed with no evidence of degradation.

Temperature and displacement data in Unit 3 were monitored.

PVNGS is a participant of the CE Owners Group (CEOG), which is scheduled to complete a generic detailed analysis for all CE plants by July 1991.

The completion of an additional plant specific analysis, which is needed to verify code compliance and to close out the Bulletin, is scheduled for October 1991.

The audit team concluded that your program for addressing the Bulletin issues is acceptable and proceeding well.

However, 13 technical
issues, 10 generic to GEOG and 3 specific to PVNGS were identified as open (see Section 3.0 of the enclosed report) and require resolution.

Conway discussed in a meeting with GEOG on May 7 and 8, 1991, and will be addressed in subsequent correspondence.

PVNGS agreed to address the 3 specific issues before their final submittal in October 1991.
