MANAGEMENT
This refers to
a meeting held with you and
members of your staff at the
NRC
Region
V office in Walnut Creek, California,
on March 15,
1991.
The
discussions
are
summarized
in Meeting Report
No. 50-528/91-12,
50-529/91-12,
and 50-530/91-12,
enclosed
herewith.
A copy of the slides
used during your
presentation
is also enclosed.
During the meeting,
we discussed
your preliminary Probabilistic Risk
Assessment
(PRA) results
and your safety
system unavailability history and
goals.
We noted the actions you have taken
and intend'o take to reduce
the
vulnerability of Palo Verde to
a total loss of feedwater event,
which your
preliminary
PRA identified as
a high risk vulnerability.
We also discussed
how these risks are coupled with safety
system unavailability.
We wish to-
reiterate that on-line maintenance. requires
a well thought out, well managed,
rational
approach.
Haintenance
that is necessary
to ensure
system
operability should
be performed promptly.
Performance of maintenance
beyond
that must
be balanced with the system unavailability risks.
We look forward to discussing with you the further actions
you are taking to
reduce
the risks identified, by your
PRA and the incorporation of those
actions
in managing your on-line maintenance/safety
system unavailability
activities.
In accordance
with 10 CFR 2.790(a),
