Enclosed are copies of the revised NRC Forms 396 (Enclosure
- 1) and 398,(Enclosure 2).
These revisions were made to reflect current operator licensing policy.
The changes to the NRC Form 396 are detailed in Enclosure 3.
NRC Form 398 changes are detailed in Enclosure 4.
