MANAGEMENT

This refers to a meeting held with you and members of your staff at the

Arizona Public Service

Company corporate offices in Phoenix,

Arizona on

'February 13, 1991.

The subjects

discussed

are summarized in Meeting Report

Nos. 50-528/91-10,

50-529/91-10,

and 50-530/91-10,

enclosed

herewith.

During this meeting

we discussed

with you the Systematic

Assessment

of

Licensee

Performance

(SALP) results for Palo Verde which we had transmitted to

you in Report Nos. 50-528/90-33,

50-529/90-53

and 50-530/90-53.

In this

meeting

we reiterated

our assessment

of your performance for the

SALP period

as

one which had improved in almost all areas

and was indicative of a greater

involvement in activities by APS senior management.

It is our view that the

task for APS managers

for the next assessment

period will be to continue

and

encourage

these

improving trends.

The areas of Maintenance,

Engineering,

and

equality Assurance

were in particular

need of additional

management

attention.

The key will be the successful

integration of these three areas with plant

operations

and the demonstrated ability to identify problems before events

occur or before they are identified by others.

You acknowledged

our SALP

. report and generally agreed with its conclusions.

Although considerable

improvement during this

SALP period has

been realized, I

would caution against

excessive

optimism.

I would strongly advise against

any

tendency to declare victory and lapse into a posture of business

as usual.

This is a particularly delicate time for APS in that the task at hand is to

still maintain

momentum and to implement the improvement programs without

being driven by major crises.

Considerable

investments of management

time and

resources

are still needed,

and the long view is necessary

to achieve the

goals originally set for the Palo Verde Project.

'During the management

meeting following discussion of the

SALP report,

we took

the opportunity to discuss

NRC management

observations.

Of particular note

was our observation of the need to focus additional

APS management

attention

on the performance of maintenance

during power operations.

We recognize that

a Preventative

Maintenance

Task Force is reviewing the overall preventative

maintenance

program.

However,

we also identified some maintenance

tasks which

had been specified to be done

on

a refueling outage frequency,

but were

done

prior to the outage while the plant was operating.

Your personnel

appear to

acknowledge the balance that must be maintained in performing maintenance

activities; however,

the manner in which it is being implemented

does not seem

consistent with this philosophy.

In addition,

a risk perspecti've

was not

apparent for the assessment

of the benefits of doing -maintenance activities in

conjunction with the associated

risk of systems

not being'available.

We

encourage

continued discussions

in this area.
