APPENDIN A NOTICE OF VIOLATION Arizona Public Service Company Palo Verde Unit 2 Docket Number 50-529 License Number NPF-51 During an NRC inspection on September 30 through November 3, 1990, a
violation of NRC requirements was identified.
In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions,"
10 CFR Part 2, Appendix C (1990), the violation is listed below:
-10 CFR Part 50, Appendix B, Criterion V, "Instructions, Procedures, and Drawings," states in part that instructions and procedures "shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."
Contrary to the above, appropriate retest and/or acceptance criteria to determine whether maintenance activities had been performed satisfactorily were not included for certain motor operated butterfly valves in Unit 2.
Specifically, k
(1)
On May 20,
- 1990, maintenance was performed on valve 2EMA-UV-145 per Work Order (MO) 374572, and appropriate retests or-acceptance criteria were not specified in the MO, resulting in the failure of the Essential Cooling Water system to operate in accordance with its design after the maintenance was complete.
(2)
On May 18,
- 1990, maintenance was performed on valve 2SPA-HV-49A per WO 385098, and appropriate retests or acceptance criteria were not specified in the WO to assure system design criteria were met.
(3)
On Hay 19,
- 1990, maintenance was performed on valve 2SPA-HV-49B per WO 385099, and appropriate retests or acceptance criteria were not specified in the WO to assure system design criteria were met.
(4). Surveillance Test 73ST-2XI07. which was conducted on valves 2SPA-HV-49A and 2SPA-HY-49B following maintenance in May 1990, lacked appropriate acceptance criteria in that leakage flow could not be either quantitatively or qualitatively determined.
This is a Severity Level IV Violation (Supplement I) applicable to Unit 2.
Pursuant to the provisions of 10 CFR Part 2.201, Arizona Public Service Company is hereby required to submit a written statement or explanation to the U.

transmitting this Notice of Violation (Notice).
This reply should be clearly marked as a "Reply to a Notice of Violation" and should include:
(1) the reason for the violation, or if contested, the basis for disputing the violation; (2) the corrective steps that have been taken and the results achieved; (3) the corrective steps that will be taken to avoid further violations; and (4) the date when full compliance will be achieved.
If an adequate reply is not received within the time specified in this Notice, an order may be issued to show cause why the license should not be modified, suspended, or revoked or why such other action as may'be proper should not be taken.
Consideration may be 'given to extending your response time for, good cause shown.
