0 APPENDIX A NOTICE OF VIOLATION Arizona Public Service Company Palo Verde Unit 1 Docket Number 50-528 License Number NPF-41 During an NRC inspection conducted on August 26 through September 29, 1990, a

violation of NRC requirements was identified.

In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1990), the violation is listed below:

10 CFR Part 50, Appendix B, Criterion XVI, states in part that "In the case of signifi'cant conditions adverse to quality, the measures shall assure that the cause of the condition is determined and corrective action taken to preclude repetition.

The identification of this significant condition adverse to quality, the cause of the condition, and the corrective action taken shall be documented and reported to appropriate levels of management."

Contrary to the above, on August 21, 1990, the Unit 1 Auxiliary Feedwater Pump AFA-POl was inoperable due to a failed relay.

The cause of this significant condition adverse to quality was not determined and corrective actions were not taken to preclude recurrence.

Recurrence of ihe failure of an identical type of relay occurred in Unit 2 on September 26, 1990.

This is a Severity Level IV violation applicable to Unit 1 (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Arizona Public Service Company is hereby required to submit a written statement or explanation to the U.

S.

Nuclear Regulatory Commission, ATTN:

Document Control Desk, Washington, DC 20555, with a copy to the Regional Administrator, Region V, and a copy to the NRC Resident Inspector office at the Palo Verde Nuclear Generating Station, within 30 days of the date of the letter transmittin~ this Notice of Violation (Notice).

This reply should be clearly marked as a 'eply to a Notice of Violation" and should include:

(1) the reason for the violation, or if contested, the basis tor disputing the violation; (2) the corrective steps that have been taken and the results achieved; (3) the corrective steps that will be taken to avoid further violations; and (4) the date when full

'compliance will be achieved.

If an adequate. reply is not received within the time specified in this Notice, an order may be issued to show cause why the license should not be modified, suspended, or revoked or why such 'other action as may be proper should not be taken.

Where good cause is shown, consideration will be given to extending your response time.
