Requalification Program Evaluation Examinations File:

we are providing the approved reference material for use in administering the upcoming operating and written examinations for evaluation of the PVNGS requalification program.
The evaluation visit is scheduled to begin at PVNGS on October 9,
1989.
Attachment A
contains signed copies of Pre-Examination Security Agreements.
These copies, signed by members of the PVNGS Training and Operations Staffs and effective 60 days prior to the examination date, are provided as required by standard ES-601, "Administration of NRC Requalification Program Evaluations",
of NUREG-1021, Revision 5,
"Operator Licensing Examiner Standards".
Attachment B contains, as required by standard'S-601, the proposed list of Operators for the Requalification Examination.
Attachment C
contains the Licensed Operator Requalification Training Objectives as requested by Reference (A).

Attachment E
is a list of topics from the last two years of Requalification Training as requested by Reference (A).
Attachment F contains an index of the approved reference material requested in Reference (A).
The reference material is contained in 15 volumes and is appended to this letter.
In discussions with Mr.
Michael Royack of your staff, it was determined that we need only supply updates to the reference material sent in support of the August 1989 Upgrade Exam.
After reviewing the material already sent we have determined that the only items needing updating are the procedures supplied in this reference set.
We are therefore submitiing one copy of 15
. volumes oz procedures (labeled as vol 'es through "0") to supplement the material already supplied.
The 15 volumes of procedures 're tabbed and fully indexed.
An additional copy of this approved reference material is being forwarded directly to EG & G Idaho Inc.,
to the attention of F. Jagger, as requested by Mr. Royack.
We are also providing, as Attachment G, proposed Reactor Operator and Senior Reactor Operator requalification written examination test items.
Pursuant to information provided by Mr. John Hannon, Chief, Operator Licensing Branch, NRC (refer to "NRC Overview'-
Exam Process" ), the NRC currently requires a
minimum of 180 examination test items
~ for each of the two sections of the written examination based upon a
total of 30 examination test items per month beginning in August 1988.
Since a final examination bank for PVNGS of 350 examination test items per section is not being supplied for this examination, security of the test items must be maintained.
To fulfillthis requirement, all test items are being sent directly to Mr. Michael Royack by copy of this letter.
