Reply to Inspection Report 528/89-18, 529/89-18, 530/89-18 File:

89-019-026 This letter is provided in response to the routine inspection conducted by Mr. C. A. Clark on April 3-14, 1989.

Based upon the results of this inspection, Mr. Clark identified a concern with our Inservice Inspection

( ISI) process for resolving all nonconforming conditions.

Although no violations or deviations were identified during the inspection, APS committed to review the appropriate procedures and issue any changes as required by July 31, 1989.

APS has completed the review of the administrative control procedures.

This review has determined that the administrative control procedure "ASME Section XI Inservice Inspection" (73AC-OXIOl) will be revised to specify conditions which can be corrected using the work control process or require an engineering evaluation.

As an interim measure to restrict elevation of a plant operating

mode, 73AC-OXIOl will be revised to reference the procedure "Mode Change Checklist" (430P-92211) such that all code rejectable indications will be dispositioned prior to entry into a mode where the indications would adversely affect'afety.

4 As long term corrective action, a Nonconformance Report (NCR) program is being developed and will include a mode restraint designation.

These NCRs will require dispositioning prior to the appropriate mode entry.

The NCR program is expected to be implemented by August 1989.
