PALO VERDE NUCLEAR GENERATING STATION LOOSE-PART DETECTION INSTRUHENTATION License No. NPF-41 Docket No. 50-528 Special Report No. 1-SR-88-004 This Special Report is being submitted pursuant to Technical Specification 3.3.3.7 ACTION "a" and Technical Specification 6.9.2'addressing the loose-part detection instrumentation being inoperable for more than 30 days. The 30 day period for returning the system to an operable status was exceeded at approximately 0010 HST on April 7, 1988.

Palo Verde Unit 1 entered Hode 2 (STARTUP) at approximately 0010 HST on Harch 8, 1988 upon completion of its first refueling outage. The inoperable portion

'of the loose-part detection instrumentation is in the circuitry associated with instrument number JSVNYE-2, which is located on the upper reactor vessel head. The inoperable status is caused by a break-up of the signal coming from the sensor.

The sensor and the hardline cable had been replaced as discussed in letter ANPP-36011-EEVB/KLH-98.05, E. E. Van Brunt, Jr. to Director of Nuclear Reactor Regulation, dated April 9, 1986. Surveillance Test 36ST-9SV02 (Vibration and Loose Parts Honitoring System Calibration Test) was performed satisfactorily on loose-part detection instrumentation in January, 1988. The reactor vessel head was removed for inspection and maintenance and reinstalled in February, 1988. On triarch 4, 1988 the loose-part detection instrumentation channel 2 problem was identified and is believed to be. either the sensor or the cable although an exact root cause has not been determined.

An authorized work control document has been prepared and will be scheduled for the next outage of sufficient duration.
