PROPOSEO IIIPOSITION OF CIVIL PENALTIES - $ 100,000 This is to inform the Commission that a Notice of Violation and Proposed Imposition of Civil Penalty in the amount of One Hundred Thousand Dollars
($ 100,000) will be issued on or about April 13, 1988 to Arizona Nuclear Power Project.
This action is based on violations of requirements in two different plant disciplines.
In the first case improper engineering and review resulted in an inoperable turbine driven Auxiliary Feedwater Pump on each unit.
In the second case operational problems including failure to follow procedures and inadequate verification resulted in, on separate occasions, an inoperable Auxiliary Feedwater Pump on Unit 2, an inoperable High Pressure Injection Pump cn Unit 1, and an inadvertent safety injection on Unit 2.
It should be noted that the licensee has'not been specifically informed of the enforcement action.
The schedule of issuance and notification is:
Mailing of Notice Telephone Notification of Licensee The State of Arizona will be notified.
April 13, 1988 April 13, 1988 The licensee has thirty days from the date of the Notice in which to respond.
Following NRC evaluation of the response, the civil penalty.may be remitted, mitigated, or imposed by Order.
