This letter refers to the special
inspection, conducted
by Messrs.
A. Johnson
and
S.
Richards of this office, on January
27 through, January
29, 1988, of
activities authorized
by
NRC License
Nos.
and NPF-74,
and to
the discussion
of our findings held with members of your staff at the
conclusion of the inspection.
The purpose of the inspection
was to review the
circumstances
leading
up to your rendering inoperable
the turbine driven
Pumps in both Units 1 and 2,
as described
in Licensee
Event Report 50-528/87-025.
Areas
examined during this inspection
are described
in the enclosed
inspection
report.
Within these
areas,
the inspection consisted of selective
examinations
of procedures
and representative
records,
interviews with
personnel,
and observations
by the inspectors.
Several violations of NRC requirements
were identified within the scope of
this inspection.
These violations will be the subject of separate
correspondence.
We are concerned that the numerous
checking
and testing requirements
of your
programs
apparently failed to identify the engineering error which led to the
inoperability of the Auxiliary Feedwater
Pumps.
Our concern is further
heightened
by a recent event at Unit 2, wherein the non-safety electric
Pump discharge
valve was erroneously left in the closed
position for several
weeks.
Although we have not yet completed our review of
this latest event,
indications are that this error also went undetected
by the
independent
checks
required
by your programs.
We wish to discuss
both events
in detail at our management
meeting scheduled for February 29,
1988.
In accordance
with 10 CFR 2.790(a),
a copy of this letter and the enclosure
will be placed in the
NRC Public Document
Room.
