This refers to the routine
on site inspection
conducted
by Messrs.
H.
S. North
and
G.
R. Cicotte of this office on May 26-29 and June 22-26,
1987
and
a
discussion
on July 17, 1987, of activities authorized
by
NRC License
Nos.
NPF-51 and NPF-65 and to the discussion of our findings held by Mr.
North with Messrs J.
R.
Bynum and J.
M. Allen and other members of your staff
at the conclusion of the inspection.
Areas
examined during this inspection
are described
in the enclosed
inspection
report.
Within these
areas,
the inspection consisted
of selective
examinations
of procedures
and representative
records,
interviews with
personnel,
and observations
by the inspectors.
Based
on the results of this inspection, it appears
that certain of your
activities were not conducted
in full compliance with NRC requirements
as set
forth in the Notice of Violation, enclosed
herewith
as Appendix A.
Your response
to this Notice is to be submitted in accordance
with the
provisions of 10 CFR 2.201
as stated
in Appendix A, Notice of Violation.
The personnel
contamination
event of May 21, 1987, involving the Unit 2 Post
Accident Sampling System
(PASS) resulted in the identification of concerns,
in
addition to the matter discussed
in the attached
The
has
been identified as
a system important to safety.
The maintenance
activity undertaken to repair
a vacuum leak failed to provide for appropriate
tests to assure
system operability prior to continuing the interrupted
Surveillance
Test.
Failure to properly test repaired or replaced
component
parts of the
PASS, following maintenance,
would appear to increase
the risk of
more serious
consequences
than occur red in this case.
Please
advise
us of the
steps
you plan to take or have taken to assure that appropriate tests of the
PASS are performed verifying that the system is fully operable
before being
returned to service.
In accordance
with 10 CFR 2.790(a),
a copy of this letter and the enclosure
will be placed in the
NRC Public Document
Room.
